Job Summary
The Hospice Bereavement Coordinator coordinates the assessment and delivery of grief counseling needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to families and caregivers. Provides support to hospice personnel coping with work related grief. Provides grief support counseling to the community as needed as requested by local government. Functions across multiple Hospice offices.

The Role
- Develops, implements and maintains bereavement plan of care for at least one family member or friend of patients who died on our hospice service.
- Delivers compassionate bereavement support via phone and in-person visits, ensuring families feel supported throughout the grieving process.
- Coordinates and supports the annual hospice memorial service, ensuring a meaningful and supportive experience for families and loved ones.
- Organizes and facilitates grief support classes in different cities throughout the year.
- Provides individual counseling to family members / friends of patients who died on our service.
- Ensures monthly, quarterly and annual contact is made to bereavement patients in accordance with Medicare Conditions of Participation guidelines and documented in patient file.
- Participates in the Interdisciplinary Team (IDT) meetings conducted in accordance with Medicare Conditions of Participation guidelines.
- Actively participates in quality assessment performance improvement teams and activities.
- Establishes a Plan of Care that addresses bereavement needs with clear delineation of services to be provided and frequency of service delivery up to thirteen (13) months following the death of the patient.
- Performs other position appropriate duties as required in a competent, professional and courteous manner.
- Provides planning and oversight for all functions of the grief support programs.
- Provide phone support: information, intakes, crisis response, and community referrals.
- Demonstrates community engagement through events and educational presentations to increase awareness and access to grief support services for the entire community.
- Demonstrates appropriate boundaries with staff, interns, volunteers and clients.
- Possess high integrity, including the maintenance of confidential information.
- Ability to effectively provide grief counseling to the family and friends of hospice patients.
- Provide bereavement information and referral services to callers from the community.
- Interact effectively with managers, employees, clients and professional colleagues.

Drivers License (TXDL)
