JOB SUMMARY

** PRN - Weekdays - must cover Temple, Marble Falls & North Austin!**

The Hospice Social Worker is assigned a group of patients to oversee social work care for both patients and families, and provides patients and family members with the education and advice needed to cope with issues which arise from various emotional or physical problems as outlined by the Texas State Board of Social Worker Examiners scope of practice. Coordination includes a variety of services including crisis intervention, bereavement, client advocacy, referrals, education, discharge planning and implementation, counseling and psychosocial assessments.

Central Texas – Temple, Marble Falls, and North Austin

SCHEDULE

PRN - Weekdays - 1 day a week is preferred

ESSENTIAL FUNCTIONS OF

THE ROLE
  • Provide highly complex education and advice to patients and family members to cope with various emotional or physical problems.
  • Develop a written plan of care for each patient, family and caregiver before providing care and services.
  • Coordinate care among team members by reviewing team conference notes, clinical notes, and telephone communication in the clinical record.
  • Assist patients and families with finding alternate means for support for basic life needs.
  • Conduct patient assessments as required by Medicare Conditions of Participation and document in the medical record.
  • Coordinate patient respite stays at local facilities, including contracting, transportation, and clinical staff for safe transfers.
  • Serve as a liaison with local community resources and staff to facilitate smooth transitions and placement of hospice patients.
  • Assist with admissions, discharges, transferring, arranging alternate levels of care, and attending the deaths of agency patients as needed.
  • Participate in the planning and support the Hospice Annual Memorial service for families of patients who passed away the past 12 months.
  • Coordinate with Texas Adult Protective Services when needed to make reports concerning patient safety or to assist in Texas APS investigations regarding patient abuse and safety.
  • Perform other position-appropriate duties as required in a competent, professional and courteous manner.

  • Use strategies and available resources for problem solving for patient/family and staff.
  • Show knowledge of the principles and methods to restore or enhance social, psychosocial, or bio‑psychosocial functioning of individuals, couples, families, groups, organizations and communities.
  • Recognize that resources may be limited and consider factors related to safety, effectiveness and efficiency in planning and delivering patient care.
  • Able to develop therapeutic relationships with patients and family and maintain appropriate boundaries.
  • Function independently in the community; able to drive own vehicle and travel within the entire office service area as needed.
  • Function as a team member and work collaboratively and independently, involving intra‑ and inter‑disciplinary work with colleagues and community.

QUALIFICATIONS
  • Education:

    Master’s degree
  • Experience:

    1 year of experience
  • Certification/License/Registration:
    Must hold one of the following licenses – Licenses:
    Clinical Social Worker (LCSW) or Master Social Worker (LMSW)
