This position is Full-Time working Monday-Friday 12pm - 8pm, seeing patients in their homes

The Infusion Nurse provides primary patient care for assigned patients, including assessments, care planning, evaluation and education of patients receiving infusion therapy and/or other nursing services in the home or alternate site setting, in accordance with care matrices and best practices.
Responsibilities include:
- Provide direct care for all assigned patients, including initiation and maintenance of communications with the care team (pharmacists, prescribers, dietitians, home patient representatives/care partners, and relevant community staff).
- Educate patients/care partners to safely administer home infusion therapy and provide additional instruction as indicated.
- Evaluate the home environment and identify interventions to render it safe and appropriate for prescribed care.
- Collaborate with team to develop and evaluate the plan of care and response to therapy.
- Develop and maintain caseload scheduling via company-approved platform and communicate scheduling changes as needed.
- Coordinate care via approved company platform.
- Participate in the nurse on-call rotation to ensure continuous nursing support services.
- Document all communications at time of service per Soleo policy.
- Demonstrate strong knowledge of therapies provided by Soleo.
- Work with relevant departments to ensure required patient documents are completed and returned to the branch office within the required timeframe.
- Ensure timely submission of plans of treatment as required by state guidelines.
- Must be able to work Full-Time, Monday to Friday, 12pm - 8pm
- Must be knowledgeable of IV access devices (IV lines, ports) and administration of infusion medications
- Must be able to work on-call, 2-3 shifts per month
- Must be able to drive up to 60 miles (1 hour) to see patients in their homes (mileage reimbursement and hourly pay apply even when driving)
- RN Degree, BSN Preferred
- Advanced certification in specialty areas (e.g., CRNI, ONS, IgRN) preferred but not required
- On-Call Requirement: This position requires participation in an on-call rotation including nights and weekends to address urgent issues and ensure continuous operations
- Administrative Oversight:
Responsible for monitoring workflows, policy compliance, and resource management - Emergency Availability: Must be available to respond promptly to emergencies, including non-standard hours
- Minimum 3 years nursing experience with infusion therapy preferred
- Current nursing license for applicable home state
- Experience in ICU and/or Cardiac Care preferred
- Proficient with Microsoft Outlook and Microsoft Office Suite
- Valid state driver’s license with no significant restrictions
