Behavioral Health Connections (BHC) in Tempe, AZ is seeking a dynamic and talented LPN Referral Management Full Time Nights to work 12 hour shifts.

• Review assessment referrals received and collaborate with RN to assess patient needs.
• Assist with coordination of patient care.
• Maintain patient records, updating databases, and ensuring accurate documentation of patient information.

• Nursing Degree, preferred Bachelors or Masters
• Licensed LPN in current State or compact license
• Current CPR, if required by a customer hospital.
• Preferred 1-3 years related work experience in Behavioral Health.
