Licensed Occupational Therapist. Open to applicants of all experience levels.
Key Responsibilities- Screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plan of care following regulatory and clinical practice standards.
- Utilize standardized assessments and evidence-based practice to support clinical interventions.
- Provide rehabilitative, skilled and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
- Administer various procedures as part of the rehabilitation plan including ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of supportive and assistive devices, etc.
- Assess patient progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
- Evaluate treatment outcomes for effectiveness; modify services as needed and make referrals based on outcomes.
- Provide consultation and counseling to patients, families, caregivers and other service providers related to physical disorders.
- Communicate effectively with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
- Provide comprehensive discharge summary of services provided per regulatory and clinical requirements.
