LPN Referral Management Full Time Nights

Behavioral Health Connections, Inc. is a provider of patient-focused comprehensive mental health services. We offer mental health evaluations and referral coordination to children, adolescents, adults and older adults in crisis 24/7 response, including all holidays.

• Review assessment referrals received and collaborate with RN to assess patient needs.
• Assist with coordination of patient care.
• Maintain patient records, updating databases, and ensuring accurate documentation of patient information.

• Registered nursing degree; preferred Bachelor or Master’s.
• Licensed LPN in current state or compact license.
• Current CPR, if required by a customer hospital.
• 1-3 years related work experience in behavioral health.
