Brief Description
Mountain Park Health Center is looking for Licensed Professional Counselors (LPC) to join our well-established and accredited team of providers. As a large Federally Qualified Health Center (FQHC) in Arizona, we are dedicated to sustaining and improving health by providing excellent, patient-centered primary care.
In this role as a Licensed Professional Counselor, you will coordinate with primary medical and behavioral health teams to ensure delivery of comprehensive care to patients. LPCs assess patients, create, and coordinate treatment plans, and oversee ongoing treatment. They can also provide traditional counseling/therapy to pediatrics and adolescents.
Essential Functions
- Provide brief, evidence-based, solution-focused behavioral interventions with active development of psychotherapeutic/psycho educational behavioral change planning for patients.
- Offer quality clinical consultation regarding mental, behavioral, emotional, and physical health issues.
- Conduct screening and functional assessment of behavioral health issues.
- Collaborate with medical providers and staff to identify patients who could benefit from behavioral health interventions and provide these services timely within the clinic.
- Develop and implement clinical pathways for specific populations.
- Facilitate groups addressing biopsychosocial issues impacting medical conditions.
- Coordinate care and access to community behavioral health services, ensuring timely feedback to providers.
- Provide crisis triage and intervention.
- Assist in de-escalating upset or frustrated patients.
- Maintain regular attendance.
- Perform other duties as assigned.
Qualifications
Minimum Qualifications:
- Current licensure in Arizona as a Professional Counselor.
- Eligibility for credentialing and contracting with insurers.
- Valid CPR card.
Preferred Qualifications:
- Training in Motivational Interviewing (MI).
- Training in Acceptance and Commitment Therapy (ACT).
