Compensation: $75–$100 per visit (salary: $70–$100 per visit)

Job Summary – The Home Health Physical Therapist evaluates, plans, directs, and administers physical therapy treatments, using prescribed interventions to restore function, prevent disability, and help patients achieve their maximum performance within their capabilities. Duties include delegating tasks, supervising assistants, and adhering to regulations and guidelines.

Desired Skills and Qualifications

• Current Physical Therapy License in the state of practice
• 2 years of PT experience
• Ability and desire to work independently
• Experience with Kinnser / Wellsky preferred
• Effective verbal and written communication & time management skills
• Valid driver’s license with proof of current insurance & reliable transportation
• Valid AZ Fingerprint Clearance Card
• Current CPR certification & proof of negative TB test
