The Hospice Clinical Pharmacist will support the operations of OnePoint Patient Care’s clinical team in our East Region and will report to the Vice President of Clinical Services.

7 days on / 7 days off:
Monday-Thursday 8:00 pm‑6:00 am, Friday 8:00 pm‑7:00 am, Saturday 6:00 pm‑7:00 am, Sunday 6:00 pm‑6:00 am.

• Ensure compliance with Federal, State and local laws and regulations, and OPPC/Client contract terms pertaining to such interventions
• Perform and document Medicare Condition of Participation (CoP)-compliant medication reviews for new hospice patient admissions
• Attend hospice interdisciplinary team / group (IDT / IDG) meetings to discuss patients and provide insights and guidance regarding the use of medications to palliate common symptoms experienced by hospice patients
• Participate in and provide clinical support to hospices by way of participation in Pharmacy and Therapeutic (P&T) committees
• Conduct and present comprehensive drug utilization evaluations which analyze medication utilization data for compliance with Medicare CoPs and opportunities to reduce avoidable medication costs via de-prescribing and therapeutic substitution
• Authorship / presentation of core OPPC clinical offerings, including webinars, company newsletter, and symptom guide book
• Provide evidence-based medication therapy advice during consults with hospice clinicians
• Update hospice formularies with consideration of hospice-specific financial targets, medication pricing methods, and utilization patterns
• Provide clinical education and training to hospice clinicians and OPPC trainees
• Provide preceptorship to Advanced Pharmacy Practice Experience (APPE) pharmacy students
• Maintain appropriate records of consultations and meetings
• Travel as necessary to provide local, regional, and national support to our clients, company pharmacies and sales and marketing efforts

• PharmD
• 2+ years hospice or palliative care pharmacy experience
• Strong familiarity with Microsoft Office suite
• Ability to multitask and prioritize tasks when faced with competing priorities
• Extensive knowledge of drug products, pharmacokinetics, symptom and disease-state management, and federal and state hospice rules and regulations
• Ability to communicate clearly in oral and written form to patients, caregivers, clinicians and administrators
• Comfort with delivery of presentations to a variety of audience types and sizes in a variety of venues
• Ability to establish and maintain working relationships with various members of health care teams
• Ability to perform data analysis and present drug utilization evaluations and other hospice pharmacy analytics as necessary
• Ability to conduct literature searches to identify, review, relate and summarize journal articles for use in clinical offerings
• Preferred Qualifications:
• Board of Pharmacy Specialties (BPS) board certification(s)
• Completion of PGY-1 or PGY-2 residency program focused on hospice or palliative care
