UCR Staff Nurse

Overview

An exciting opportunity has arisen to join the Shropshire, Telford & Wrekin Urgent Community Response & Virtual Wards service as a Staff Nurse. The post will form part of a seven day service based within one of the Trust's locality teams. The service provides patients who are experiencing an acute change in their health a safe and effective alternative to hospital admission, supported by a mixture of face to face visits and digital technology.,

As a service which is at the forefront of the NHS Long Term Plan agenda to develop Urgent Community Response services and improve our local system responsiveness to managing community health crisis response services, the role on offer represents an ideal opportunity for a Registered Staff Nurse to join the established Multidisciplinary Team and continue to help shape our future.

Your role will be to ensure the continuation of a seamless service to avoid unnecessary hospital admissions.

The successful applicant will require the appropriate skills and knowledge outlined in the person specification, and be registered with the NMC, as well as share the Trusts vision to provide assessment and delivery of comprehensive, holistic, evidence-based nursing and care support for the management of acutely unwell patients referred to the Shropshire UCR.
