Carry out Care Act assessments and reviews for adults with learning disabilities and autism.

Work strategically to maximise healthcare funding and resources for service users.

Develop and oversee personalised care plans
, working closely with individuals and their families.

Provide advocacy and support to ensure service users’ rights, choices, and wellbeing are always prioritised.

Maintain accurate, confidential case records and documentation.

Collaborate with multi-disciplinary teams to coordinate holistic, effective support.

Participate in supervision, team meetings, and case discussions to drive continuous improvement.
