Location:Telford (with reimbursed travel to other local areas for learner pick-ups and occasional travel to other sites for training and development)
Reports to:Teaching, Learning and Assessment Coordinator
Contract:Full-time
Hours:40 hours per week
Working Pattern:Monday to Friday
The Change Mentor a vital role in supporting young people aged
16–24
who face complex educational, social, and personal barriers. Working both1:1 and in small groups
, on-site and in the community, the post holder will deliver tailored mentoring, learning, and wellbeing support to help young people re-engage, progress, and move towards positive outcomes.
- Uphold the organisation’s
values, ethos, and professional standards
at all times. - Build and maintain positive, professional relationships with learners, families, colleagues, and external professionals.
- Follow and uphold
safeguarding and child protection procedures
in line with organisational policy. - Deliver high-quality youth and mentoring support to help young people overcome educational and personal barriers.
- Contribute to a
safe, supportive environment
where young people feel confident to seek help. - Carry out learner pick-ups and drop-offs when required, ensuring appropriate vehicle insurance, tax, and MOT are in place.
- Maintain accurate and timely records using organisational systems.
- Comply with
Health and Safety at Work
requirements.
- Support learners
on‑site, in the community, and in their homes
, working 1:1, 2:1, and in small groups. - Plan and deliver
bespoke workshops
focusing on life skills, careers, employability, and wellbeing. - Deliver high-quality teaching and learning activities that support progress, engagement, and attainment.
- Adapt and differentiate sessions to meet individual needs, including learners with
SEND
. - Assess learners’ progress and provide high-quality verbal and written feedback.
- Develop engaging, differentiated learning resources to support accessibility and motivation.
- Work collaboratively with the
SEN Coordinator
to prepare, implement, and review Individual Education Plans and EHCP outcomes. - Support learners to identify and access
work experience, employment, and progression pathways
. - Contribute to transition planning and onward progression for learners leaving the service.
- Maintain up‑to‑date knowledge of effective
SEND strategies
and inclusive practice. - Provide input into educational reports and SEND documentation when required.
- Participate in required
training, CPD, engagement activities, and meetings
.
Job Type: Full-time
Pay: £26,000.00-£27,516.66 per year
