Overview
Registered Nurse - Home Health Care - Full Time - Days. Under general supervision of the nursing supervisor, utilizes the knowledge and application of nursing principles to independently provide direct patient care in the home. Assesses need for other allied health care services such as home health aide services, physical therapy, occupational therapy, dietetics, speech-language pathology, and social work, to ensure an integrated home health care delivery system.
Performs other related duties and projects as assigned.
- Provide direct patient care in the home setting in accordance with nursing principles and patient needs.
- Assess the need for allied health services (home health aide, physical therapy, occupational therapy, dietetics, speech-language pathology, social work) to support integrated home health care delivery.
- Perform duties as assigned by supervisor and participate in care planning.
- Requires graduation from an accredited Registered Nurse program.
- Current licensure by the State of Michigan as a registered nurse.
- A minimum of nine months to one year clinical nursing experience preferred.
- Ability to effectively communicate with patients, families, and health care personnel.
- Computer literacy to electronically document patient care.
- Strong interpersonal and customer service skills; ability to interact with individuals from diverse backgrounds.
- Must possess a current driver’s license, satisfactory driving record, and reliable transportation.
- Must be able to lift, carry, transport and move clients, if needed.
