Description of Position

The Community Support Program (CSP) is now hiring full‑time Community Outreach Case Workers to join our teams in Bristol and Plymouth counties. Our program provides short‑term, intensive, case‑management support services to clients with a history of behavioral health and/or substance‑use disorders. As a CSP worker you will assist clients to reach their short‑term goals and live independently and successfully in the community.

By connecting clients to community resources, you can help them achieve higher quality of life and reduce the need for 24‑hour levels of care.

Strongly prefer multilingual candidates able to speak and read either Spanish, Portuguese, Haitian Creole, or Cape Verdean Creole.

Responsibilities
  • Establish and maintain relationships with clients, providing contact and support through outreach in the community, client’s home, inpatient treatment facility, or other designated location.
  • Collaborate closely and effectively with all internal/external team members and natural supports.
  • Conduct comprehensive needs assessment and create individualized action plan (IAP).
  • Connect client to benefits, housing, health care and other resources needed to support client goals; provide transportation to essential medical and behavioral health appointments.
  • Foster empowerment, recovery, and wellness, including linkages to recovery‑oriented, peer support and/or self‑help supports and services.
  • Document all encounters, and contacts made with clients and collaterals and maintain client electronic medical record.
  • Obtain insurance authorizations and check eligibility on a daily basis.
  • Actively participate in staff meetings, in‑service training, and all other agency‑sponsored events.
Requirements
  • Bachelor’s degree in psychology, social work, sociology, or a related field, or a minimum of 3 years related experience.
  • Must have good time‑management skills, verbal and written communication skills, and interpersonal skills.
  • Personal, insured vehicle with a clean driving record (driving record will be verified).
Preferred Qualifications
  • Multilingual (able to speak and read Spanish, Portuguese, Haitian Creole, Cape Verdean Creole, etc.).

A short‑term program that provides intensive case‑management services to clients considered to be “at risk” within the community.
