Position: CHC Social worker (Contract)
CHC Social Worker / Practitioner

Hybrid: 2 days per week in the office

The Role

We are seeking an experienced and knowledgeable practitioner to join a multi-disciplinary team as the Local Authority representative for NHS Continuing Healthcare (CHC) assessments.

This is a specialist role requiring in-depth knowledge of CHC eligibility, confident application of the Decision Support Tool (DST), and a strong understanding of the lawful limits of Local Authority provision under Section 22 of the Care Act 2014.

You will play a crucial role in ensuring robust, lawful and defensible CHC decision-making across a range of settings including hospitals, community placements, and residential environments.

Key Responsibilities

Complete CHC assessments and Decision Support Tools (DST) to a high professional standard

Represent the Local Authority perspective as a core member of the Multi-Disciplinary Team (MDT)

Constructively challenge within MDT discussions to ensure evidence-based and lawful decisions

Apply detailed knowledge of Section 22 Care Act 2014 when determining LA responsibilities

Identify and contribute to joint funding arrangements where appropriate

Work across hospital, community and care home settings

Maintain accurate, defensible records and reports in line with national CHC guidance

Essential Requirements

Qualified Social Worker / experienced practitioner with relevant background

Extensive experience completing CHC assessments and DSTs

In-depth understanding of NHS CHC eligibility criteria and National Framework

Strong working knowledge of Section 22 Care Act 2014 and lawful limits of LA provision

Experience working within MDT environments

Confident in professional challenge and decision-making

Excellent report writing and evidence-gathering skills
