LPN-PACE

PACE, a member of Empath Health is seeking a compassionate Licensed Practical Nurse (LPN) to join our interdisciplinary PACE team in Pinellas Park, FL
. In this role, the Licensed Practical Nurse will assist in providing care in the clinic setting as well as in the home.

The Program of All-Inclusive Care for the Elderly (PACE) is a unique care model designed to help older adults remain safe and independent in their own homes.

  Collaborate closely with physicians, social workers, therapists, and other clinicians in a proactive care model focused on prevention, wellness, and holistic well-being.

• Take responsibility for evaluating and improving the quality of participant care and services provided.
• Document in the medical record, to include significant changes in participant status.
• Update care plans with changes in participant status.
• Complete documentation of all visits (including episodic, routine, phone calls, etc) against the corresponding care plan elements.
• Review medical record to ensure that appropriate information such as labs, specialist reports and diagnostic test results are captured in the medical record.
• Identify participant’s overt problems/needs and set priorities.
• Identify problems requiring further referral and/or follow up.
• Interpret and record latest diagnostic results.
• Collaborate with the RN on a plan of care based on nursing process which incorporates the plans of other disciplines and continuing care needs.
• Ensure care provided conforms to accepted practice standards; provides correct treatments/procedures and other care as prescribed and according to participant care standards.

• Graduate of a school of professional nursing program required.
• Valid CPR certification from the American Red Cross or American Heart Association.
• A minimum of one year’s experience in working with the frail or elderly population required.
