• Reviews reports on Current Core Measure Hospitalizations and Re‑admissions daily.
• Coordinates the Readmission Prevention Committee Meetings, identifies recent readmission cases, reviews for improvement opportunities, creates and sends out meeting agenda, and completes meeting minutes.
• Participates in in‑patient visits and post‑discharge phone calls, utilizing data reports to identify patients with Core Measure diagnoses.
• Visits patients in the hospital, provides education on disease process and medications, and collaborates with Care Management to address discharge needs.
• Collaborates with Respiratory Therapy and Cardiopulmonary Rehab Teams when appropriate.
