Support Partners provide hands-on, individualized support to underserved youth and their families through structured in-home and community-based interventions. The Support Partner conducts home visits to offer behavioral coaching, emotional regulation techniques, and academic support while also completing intake assessments to identify family strengths and needs. Support Partners help caregivers and youth manage impulsive behaviors, improve social skills, and build emotional resilience.
Weare looking for a Support Partner who must have:
- Bachelor’s degree in social work or a related area of study from an accredited college or university required.
- Minimum of three (3) years’ experience working in the social work field required
- Experience navigating the education system and behavioral health and training, coaching and/or serving, as a mentor to other families preferred
- Experience working with diverse populations and collaborating with a wide range of service providers. Educational background experience preferred.
- Must have valid driver’s license, ability to use personal vehicle for work purposes, and appropriate vehicle insurance.
- Work independently and as a team member to create innovative ways to meet goals and provide a high level of service that is engaging and meets the needs of the participants.
- Manage a caseload of 10-15 families
- Conduct weekly or bi-weekly home visits to offer behavioral coaching, emotional regulation techniques, and academic support while also completing intake assessments to identify family strengths and needs
- Connect families to essential resources such as educational tools, mental health services, financial aid, and community programs, responding to urgent needs promptly.
- Integrate CASEL, Safety Planning, and Trauma Informed Care into service provision.
- Develop a service plan based on positive youth development and principles of Motivational Interviewing.
- Perform other administrative tasks as assigned and assist with maintaining proper document storage and auditing.
- Meet data collection, contract requirements and contract deadlines.
- Provide progress reports as required.
- All other duties as assigned
- Develop relationships with public and private service providers as well as with the education and school system, and other stakeholders.
- Develop expertise knowledge of the different social service systems including eligibility requirements, services offered, and the application process.
- Coach caregivers to navigate systems that influence or govern essential needs such as educational tools, mental health services, financial aid, and community programs; refers to appropriate services and assist those overcoming barriers to accessing services.
- Accept all cases assigned by Program Director and meet with youth in their primary residence and with identified parents in sufficient time
- Able to drive daily to nearby locations for meetings or visits.
