RN - Home Health

Day shift: 5x8-Hour (08:00 - 17:00) with varying day/shift hours based on the needs of the Home Care office.

We are seeking an experienced Skilled Home Care Registered Nurse with strong infusion and home health experience. The ideal candidate will be responsible for providing high-quality nursing care to patients in their homes, focusing on cardiopulmonary assessment, wound care, and managing complex medical diagnoses.

• Perform patient assessments and evaluations.
• Administer IV medications and initiate IV therapy.
• Provide wound care management, including dressing changes and wound vac care.
• Manage and care for patients with high-risk conditions such as heart failure, COPD, and post-operative needs.
• Conduct skilled nursing visits, ensuring compliance with Medicare and insurance requirements.
• Document patient care in the electronic health record system.
• Collaborate with interdisciplinary teams, including social services and pharmacy.

• Must have a minimum of 1 year of skilled home care experience.
• OASIS training and experience required.
• Valid Florida RN license or compact license required.
• Must have current BCLS certification.
• Experience with various medical procedures including CCU/CARDIAC experience, IV skills, and wound care.
• Must have a valid driver’s license and a reliable vehicle for home visits.
