DaVita, the provider, partner, and employer of choice, is seeking a Clinical Coordinator who will deliver compassionate care in an outpatient dialysis center, making a difference in the lives of patients with end‑stage renal failure or chronic kidney disease.

• At least 6 months dialysis experience required.
• Coordinate ESRD patient care and services; monitor trend lab values, vital signs, weight, and outcome metrics.
• Ensure patient safety, comfort, and well‑being; collaborate with the clinical team.
• Participate in training at the home clinic or a designated training facility.
• Potentially float to various clinics during and after training.
• Maintain flexible schedule working mornings, evenings, weekends, and holidays.

• Build meaningful, long‑term relationships with patients and families in an intimate outpatient setting.
• Work within a positive, supportive interdisciplinary team led by staff RNs.
• Provide care to patients with multiple comorbidities, leveraging broad nursing skills.
• Oversee patient observations, measurements, and machine setup with PCTs.
• Engage daily with patients in a fast‑paced environment.

• Current RN license in the state of practice.
• Current CPR certification.
• At least 18 months of registered nursing experience.
• Dialysis Charge RN readiness approval by Clinical Nurse Manager or RN‑licensed FA/GFA.
• Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) preferred.
• Experience in ICU, CCU, ER, or Med‑Surg preferred.
• Associate Degree in Nursing (ADN) required; BSN preferred; diploma may substitute.
• Completed CMS‑required training for water treatment system operations.
• Supervisory experience preferred; willingness to supervise required.
• Basic computer skills in MS Word and Outlook.
