Registered Nurse - Primary Care Mental Health Integration

James A Haley is excited to recruit for a Registered Nurse within our Nursing Mental Health department.

The Registered Nurse will be assigned to our Primary Care Mental Health Integration (PCMHI) program. The positions report directly to the nurse manager of outpatient mental health.

Duties listed but not limited to:

* The RN Care manager will be conducting initial comprehensive assessments of mental and behavioral health concerns for veterans referred to PCMHI.

* Providing follow-up of patient progress and treatment adherence with reassessment and communication with their mental health and primary care providers for veterans enrolled in PCMHI's anti-depressant monitoring program. Panel size can be up to 100 Veterans.

* Delivering brief, behavioral interventions to veterans with mild to moderate mental health conditions including depression, anxiety, and stress.

* Care Management services are protocol-driven and our PCMHI program uses validated assessment tools in Behavioral Health Lab (BHL) for data entry and tracking.

* Administrative oversight is provided by the RN manager. RNs will also work closely with our large team of PCMHI psychologists and the Patient Aligned Care Team (PACT) teams.

* The RN will demonstrate extensive oral and written communication skills: contacts patients through written correspondence and telephone calls, and conducts follow-up interviews and creates and maintains forms, surveys, instruments, and materials for proper coordination of the different treatment modules available.

* Provides communication and collaboration with families and /or other concerned individuals as appropriate (given confidentiality concerns) to support the Veterans Recovery goals.

* The RN must be able to fully participate in Preventive Management of Disruptive Behavior requirements (Part 1 & Part
2) and respond to other areas of that require PMDB-related intervention, including therapeutic communication skills in managing care.

* The RN possess the knowledge, skills, and experience necessary to effectively apply all aspects of the nursing processes within a collaborative, interdisciplinary mental health team setting.

* The RN may be provided training in other areas of Nursing Mental Health service and support staffing needs in those areas.

* The RN care manager will be required to work 20 percent as a depression case manager (DCM).

* No show calls, process improvement projects and other duties as assigned by Nurse Manager.

* Assist with medical emergencies in the clinic as needed.
