Job Description
Effectively represents the organization and the clinical perspective to governing bodies and external audiences, ensuring a consistent, principle-driven brand for clinical practice across the system. Leads the execution of the strategic vision for care management across the organization, enhancing the consumer experience and deploying Care Management resources effectively. Develops strategies to reduce length of stay, readmissions, and clinical denials while simplifying daily workflows and supporting improved governance and accountability structures.
Implements policies to decrease variability in care management practices and embed evidence-based care transition practices. Advises and supports the standardization of care management roles, responsibilities, job descriptions, and accountability metrics across the enterprise. Sets care management benchmarks and tracks performance across markets, providing data analysis and monitoring action plans when benchmarks are not met. Provides guidance and actively participates with facility staff to identify, implement, or enhance care management programs consistent with corporate and facility strategies.
Determines proactive strategies and methods necessary for a high-performance culture related to care management and social work. Disseminates information accurately and timely with multiple audiences and leads regularly scheduled communication sessions with facility care management leadership. Identifies process improvement opportunities and develops, implements, monitors, and revises action plans. Collaborates with leaders from Utilization Review, Population Health, and Post-Acute, as well as other members of the clinical team, to ensure care coordination across all levels of care.
Analyzes current and future regulatory/accreditation requirements, practice trends, and legislative issues to forecast potential impact on care management. Ensures the incorporation of best evidence into care management policies and practices systemwide and establishes mechanisms for sharing best practices across the system. Performs other duties as assigned.
