$38,264.07 - $42,487.64 Annually $1,471.69 - $1,634.14 Bi-weekly
Position Summary
This is a career service position located in the APD regional offices with primary responsibility for the review of new Adult Day Training (ADT) licensing applications and renewal applications, issuing of licenses, the ongoing monitoring of ADT licensed facilities and the tracking of all licensure and monitoring activities.
The Work You Will Do
Receive new licensing application and review for completeness. Provide feedback to the applicant in writing for any errors, omissions or additional documentation required within 30 days of receipt of the licensing application.
Complete review of the application and approve or deny the application within 90 days of receipt of a complete application. This includes issuing the ADT Licenses within required time frames.
Track all licensed ADT and monitoring visits in the appropriate, approved tracking system to reflect correct ADT locations, compliance, and licensed programs.
Conduct a Quarterly monitoring visit for each ADT using the ADT Monitoring Checklist.
Create and update all licensing information in APD systems, including iConnect.
Document all violations and deficiencies with the appropriate corrective action, including Corrective Action Plans, Regional Operations Manager Letter, Notices of Non-Compliance, Administrative Action, or Provider Agency Action Request Form (PAARF).
Complete the Central Abuse Hotline Records Search (CAHRS) form for each licensing application received to ensure the licensee or applicant does not have verified findings of abuse, neglect, or exploitation.
Ensure that all personnel records are reviewed prior to initial licensure and annually as part of the license renewal process using the Personnel Record Checklist.
If the setting/program is applying to be licensed to serve individuals with mobility issues, verify at time of initial site visit or renewal site visit that the facility meets the accessibility needs of the current attendees. Notify deficiencies noted in the applicant’s documentation or observation during the site visit in writing.
Assist with disaster planning activities as assigned.
Perform other related duties as assigned.
