Bilingual Analyst, Case Management – Field – Must reside in East Tampa Florida
Candidate must be located in East Tampa or surrounding areas (preferred zip codes 33617, 33612).
This role will require 50–75% travel for face‑to‑face visits with members in the above areas. Schedule is Monday‑Friday, standard business hours.
We are seeking a self‑motivated, energetic, detail‑oriented, highly organized, tech‑savvy Case Management Coordinator to join our team. The Coordinator will manage a caseload that includes supportive and medically complex members.
The Coordinator will utilize critical thinking and judgment to collaborate and inform the case management process, facilitating appropriate healthcare outcomes for members by providing care coordination, support, and education through the use of care management tools and resources. Conduct comprehensive evaluations of members using care management tools and information/data review.
Coordinate and implement assigned care plan activities and monitor care plan progress. Conduct multidisciplinary reviews to achieve optimal outcomes. Identify and address quality of care issues through established channels. Use negotiation skills to secure appropriate options and services necessary to meet the member’s benefits and healthcare needs.
Use influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum health.
Required Qualifications- Bilingual in English & Spanish
- Must reside in East Tampa FL (Applicable Zip Codes: 33617 or 33612)
- Willing and able to travel 50–75% of their time to meet members face‑to‑face in Tampa or surrounding areas.
- At least 1 year of experience in behavioral health or a long‑term care setting
- Ability to multitask, prioritize, and adapt to a fast‑paced changing environment.
- Effective communication skills, both verbal and written.
- Managed Care experience.
- Computer proficiency in Microsoft Word, Excel, and Outlook.
- Case management and discharge planning experience.
- Bachelor’s Degree required, with a field of study in Social Work, Behavioral Health, or Human Services preferred.
- Master’s degree preferred.

The Typical Pay Range For This Role Is$21.10 - $36.78 per hour
This pay range represents the base hourly rate or base annual full‑time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short‑term incentive program in addition to the base pay range listed above.
