A company is looking for a Care Navigator to work remotely.
Key Responsibilities- Determine client needs and connect them to appropriate EAP services
- Complete EAP Intake Forms and manage client case assignments
- Maintain communication with clients and affiliates regarding service requests
- High School diploma required;
Bachelor or Associate's degree preferred - Experience with IT applications and database systems
- Professional call center or scheduling experience is a plus
- Strong commitment to customer service
- Familiarity with Microsoft Outlook and Office programs
