The postholder will play an important role within a PCN to proactively identify and work with patients, including those with long‑term conditions, to provide coordination and navigation of care and support across health and care services.

They work closely with GPs and practice teams to manage patients, acting as a central point of contact to ensure appropriate support is made available. The role will be involved in attending community clinics and clinics in GP practices to provide proactive health checks and screening and working with the PCN team and practice teams within primary care.

Carry out and co‑ordinate quality health outcomes. This may include but not exclusively the following based on training and experience: blood pressure monitoring, phlebotomy taking blood tests, vaccinations, act as a chaperone if required.

Undertake clinical activities identified by the clinical team in accordance with agreed written protocols.

Understand when to report changes in a patient’s condition.

Make clear and accurate entries into a patient’s records when appropriate.

Work as an effective team member.

Liaise with other team members and patients.
