Overview

Specialist Occupational Therapist for the Community Neuro Rehab Team - Band 6
Full Time, 8.5 months Fixed-Term Contract. Part-time hours (28.5) may be considered with an increase to the contract length (12 months).

This is an excellent opportunity to work within our established CNRT. You will carry out occupational therapy assessments, plan and deliver treatment and offer education and support to patients and their families, both in an in-patient setting and within a variety of community settings. You will be expected to practice autonomously but work as a part of an integrated multi-disciplinary team.

You will be expected to carry a complex clinical caseload and be required to delegate work to support staff.

  • To manage a clinical caseload using evidence-based and client centred principles to assess, plan, implement and evaluate interventions.
  • To undertake clinical duties as an autonomous practitioner and be professionally and legally accountable for all aspects of own practice.
  • To be responsible for maintaining own records of intervention in care pathways and for completing own contact data.
  • To conduct and record comprehensive assessments of patients' needs within the home environment as indicated.
  • To analyse complex problems and assist clients to determine realistic and achievable goals, which are measurable and appropriate to their current and longer-term needs.
  • To plan individual treatment programmes which reflect the clients’ goals. This may include specific specialist occupational therapy assessments (such as complex cognitive or perceptual assessment, assessing activities of daily living).
  • To undertake clinical risk assessment on a daily basis on home visits, in isolated assessment areas and when lone working.
  • In conjunction with the MDT, to set goals which are reviewed regularly.
  • To monitor, evaluate and modify treatment in order to measure progress and effectiveness of interventions.
  • To prepare relevant specialist reports.
  • To attend relevant clinical meetings.
  • To liaise closely with specialist community services and complete ongoing referrals as required.
