Specialist Occupational Therapist – Community Neuro Rehab Team

Band 6
• Full Time, 8.5 months Fixed-term Contract. Part-time hours (28.5) may be considered with an increase to the contract length (12 months)

This is an excellent opportunity to work within our established Community Neuro Rehab Team. You will carry out occupational therapy assessments, plan and deliver treatment and offer education and support to patients and their families, both in an inpatient setting and within a variety of community settings.

You will practice autonomously but work as part of an integrated multi-disciplinary team. You will carry a complex clinical caseload and delegate work to support staff.

Main duties of the job
  • Manage a clinical caseload using evidence-based, client-centred principles to assess, plan, implement and evaluate interventions.
  • Act as an autonomous practitioner and be professionally and legally accountable for all aspects of own practice.
  • Maintain accurate records of intervention in care pathways and complete own contact data.
  • Conduct and record comprehensive assessments of patients' needs within the home environment as indicated.
  • Analyse complex problems and assist clients to determine realistic and achievable goals that are measurable and appropriate to their current and longer-term needs.
  • Plan individual treatment programmes that reflect the clients' goals. This may include specific specialist occupational therapy assessments such as complex cognitive or perceptual assessment and activities of daily living.
  • Undertake clinical risk assessment on a daily basis during home visits, in isolated assessment areas and when lone working.
  • In conjunction with the MDT, set goals that are reviewed regularly.
  • Monitor, evaluate and modify treatment in order to measure progress and effectiveness of interventions.
  • Prepare relevant specialist reports.
  • Attend relevant clinical meetings.
  • Liaise closely with specialist community services and complete ongoing referrals as required.

Person Specification Education & Qualifications
  • BSc Honours Occupational Therapist or equivalent
Knowledge
  • Evidence of CPD
Experience
  • Previous experience at junior level within the clinical field of neurology
  • Knowledge of EC manual handling guidelines
  • Previous experience of community working
  • Previous experience of being part of an MDT
Skills and abilities
  • Ability to manage own caseload and prioritise work.
  • Ability to communicate effectively with people who have barriers to understanding.
  • Ability to communicate with patients, carers and other professionals in health/social care and voluntary agencies.
  • Ability to assess and plan treatment using a problem solving approach.
Work related circumstances
  • Ability to travel to various locations between Tameside and Glossop
  • Registration with the HPC
