We are seeking a Registered Nurse (RN) to join our Home Health Agency on a PRN (as needed) basis in the Twinsburg/Kent area. The RN will be responsible for providing skilled nursing care to patients in their homes, assessing their health status, developing care plans, and collaborating with other healthcare professionals to ensure optimal patient outcomes.

• Valid RN license in the state of practice
• Minimum of 1 year of nursing experience, preferably in a home health setting
• BLS certification
• Strong communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
• Reliable transportation and a valid driver's license

• Conduct initial assessments and ongoing evaluations of patient needs
• Develop individualized care plans in collaboration with the patient, their family, and other healthcare providers
• Administer medications and treatments as prescribed
• Educate patients and their families on disease prevention and management
• Monitor patient progress and update care plans as needed
• Work closely with other healthcare professionals to coordinate patient care
• Maintain accurate and up-to-date patient records
