* Provide
** one-on-one skilled nursing care
** in patient homes, including:
* Wound care, Catheter care, Medication administration, Symptom management
* Perform skilled observation, assessment, and evaluation
* Educate and support patients and families/caregivers
* Collaborate with interdisciplinary healthcare teams
* Maintain accurate clinical documentation

Qualifications
* ** Active Washington State LPN license** (or in place before first day of work)
* ** Home Health experience preferred but not required
*** Strong
** critical thinking
** and ability to work independently
* ** Current BLS certification (AHA)**
* ** Valid driver’s license
** with insured, reliable vehicle
