** Essential Duties**
* ** Monitor
* * chronic condition patients and track symptoms/vital signs
* ** Manage
* * medications, address side effects, and optimize symptom control for palliative care
* ** Educate
* * patients on disease management, self-care, and end-of-life care options
* ** Develop
* * and update individualized care plans, including palliative goals and comfort care
* ** Provide
* * health coaching, lifestyle guidance, and emotional support for patients and families
* ** Follow
* * select Home Health patients who need provider oversight
* ** Complete
* * up to 7-8 patient visits a day, in various home settings and living environments
* ** Offer
* * telehealth services for follow-ups and remote symptom monitoring
* ** Collaborate
* * and coordinate with specialists, palliative teams, and hospice providers for integrated care
* ** Advocate
* * for patient access to resources, including palliative and comfort care services
* ** Maintain
* * accurate patient records and document care plans, including palliative interventions
* ** Conduct
* * health risk assessments, with an emphasis on quality of life and symptom management
* ** Address
* * psychosocial and spiritual needs, facilitating advanced care planning discussions
** Requirements and Qualifications
*** Nurse Practitioner (ARNP) licensure of one year or more
* Possess current Medicare Part-B credentialing, or have the ability to do so
* *** Prior Palliative and CCM (Chronic Care Management) experience preferred, but not required
**** Be a licensed driver with an insured automobile in good working order
