  • Perform medication reconciliation and communicate concerns to providers
  • Coordinate care between hospitals, SNFs, home health agencies, and providers
  • Obtain vitals, patient histories, and clinical updates (within scope of license)
  • Educate patients and families on care plans, medications, and next steps
  • Document thoroughly and timely in the EMR
  • Collaborate with physicians, APPs, nurses, and facility staff
  • Travel to post-acute facilities and patient homes throughout Washington State
Role Eligibility

This role may be filled by:

  • Licensed Practical Nurse (LPN)
You’ll Thrive Here If You Are
  • Organized, proactive, and patient-focused
  • Comfortable working independently in the field
  • Strong with communication and follow-through
  • Experienced in post-acute, home health, or care coordination (preferred, not required)
  • A veteran or military-affiliated professional (encouraged to apply)
  • Full-time, weekday day shift
  • Occasional evenings or weekends with advance notice
  • Field-based role with regional travel
Compensation
  • Medical Assistant: $27.00 – $29.00/hour
  • Licensed Practical Nurse (LPN): $32.00 – $38.00/hour
  • Pay based on experience, licensure, and scope of role.
  • Medical, dental, and vision insurance
  • 401(k) with employer contributions
  • Paid time off and paid holidays
  • Supportive, collaborative team environment

If you’re passionate about improving patient outcomes during critical transitions of care—and want a role with autonomy, purpose, and strong clinical support—we’d love to hear from you.
