We’re Hiring:
Transitional Care Management (TCM) – Medical Assistant or LPN
Pacific Medical Specialty Group, LLC is growing—and we’re looking for a compassionate Medical Assistant or Licensed Nurse (LPN) to join our Transitional Care Management (TCM) team.
This role is ideal for clinicians who enjoy working independently in the field while staying closely connected to a collaborative care team. You’ll play a critical role in supporting patients as they transition from hospitals or skilled nursing facilities back to home and other care settings.
Why This Role Matters
Transitions of care are one of the most vulnerable times for patients. In this role, you’ll help close gaps in care, reduce readmissions, and ensure patients feel supported, informed, and connected after discharge.
ResponsibilitiesWhat You’ll Do
- Conduct post-discharge outreach and follow-up with patients and caregivers
- Perform medication reconciliation and communicate concerns to providers
- Coordinate care between hospitals, SNFs, home health agencies, and providers
- Obtain vitals, patient histories, and clinical updates (within scope of license)
- Educate patients and families on care plans, medications, and next steps
- Document thoroughly and timely in the EMR
- Collaborate with physicians, APPs, nurses, and facility staff
- Travel to post-acute facilities and patient homes throughout Washington State
This role may be filled by:
- Certified Medical Assistant
, or - Licensed Practical Nurse (LPN)
- Organized, proactive, and patient-focused
- Comfortable working independently in the field
- Strong with communication and follow-through
- Experienced in post-acute, home health, or care coordination (preferred, not required)
- A veteran or military-affiliated professional (encouraged to apply)
- Full-time, weekday day shift
- Occasional evenings or weekends with advance notice
- Field-based role with regional travel
- Medical Assistant: $27.00 – $29.00/hour
- Licensed Practical Nurse (LPN): $32.00 – $38.00/hour
- Pay based on experience, licensure, and scope of role.
