LPN/LVN – Home Health / Hospice

Shift: 8-hour shifts

Contract Length: 13 Weeks with possible extension

The LPN/LVN provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse and in accordance with the physician’s plan of care. This role supports home health and hospice patients, ensuring high-quality, compassionate, and cost-effective care while coordinating with an interdisciplinary team.

• Assist the RN in assessing patient needs and delivering nursing care per the established care plan.
• Participate in planning, coordinating, and implementing patient care.
• Document clinical findings, interventions, communication, and outcomes accurately.
• Educate patients and families on self-care techniques and monitor their response to care.
• Follow infection control and OSHA safety standards.
• Support end-of-life care activities for hospice patients.
• Assist with admissions, discharges, and transfers.
• Participate in on‑call rotation as needed.
• Maintain professional boundaries and ensure a safe working environment.
• Perform other duties as assigned.

• Current LPN/LVN license in the practicing state
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto liability insurance
• Minimum 2 years of clinical experience (home health or hospice preferred)
• Ability to use Homecare Homebase (EMR)
• TB vaccination record and Hepatitis B consent/declination
• Fluent in English with strong communication skills
• Strong attendance and professionalism

Graduate of an accredited LPN program or School of Vocational Nursing.
