Cardio Renal Vision (CRV) is a well-established, physician-led multispecialty group dedicated to providing exceptional cardiology and nephrology care for medically complex patients in skilled nursing facilities. We are currently seeking a motivated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA-C) to join our team in the Tacoma, WA area. As a CRV specialist, you will practice with a high level of autonomy while benefiting from one-on-one mentorship and ongoing support from our supervising Cardiologists, Nephrologists, and support team. You will participate in a robust onboarding program designed to build confidence and ensure clinical excellence.

Preferred Qualifications
- Prior experience in skilled nursing, internal medicine, family medicine, geriatrics,
cardiology
, or nephrology is strongly preferred. - Proven ability to collaborate with facility staff, including nursing directors, executive leadership, and other advanced practice providers.
- Commitment to ongoing clinical education and a proactive approach to patient care.
- Compassionate, patient-centered approach to managing chronic and acute conditions in a vulnerable population.
Primary Responsibilities
- Deliver high-quality Cardiology and Nephrology care to Skilled Nursing residents
- Collaborate with facility and primary care leadership
- Provide education and support to facility staff on relevant clinical topics
- Meet and maintain monthly encounter goals
- Serve as a clinical resource in cardiology and nephrology for the care team
- Complete documentation accurately and in a timely manner
