Nurse; Behavioral Health Care Facilitator

About the Position:

This position is located in the Managed Care Department at the Madigan Army Medical Center in Tacoma, WA. This is a Direct Hire Solicitation.

• Perform independent, comprehensive telephone and clinical triaging in the primary care setting, supporting primary care providers in their treatment of patients with behavioral health conditions.
• Monitor patient progress and symptom severity by conducting a structured telephonic interview and using validated tools.
• Assist clinic with patient and staff education and provide individual and small group training in order to increase referrals and promote understanding of the role of the behavioral health care facilitator.
• Collaborate with patients to develop SMART self-management goals to assist patients with symptom improvement.
• Follow quality improvement and internal control activities for the ambulatory care setting to include risk appraisals, conducting chart reviews, identifying trends and reporting corrective actions.
• Coordinates patient referrals to support services provided through the internal and external community, or the TRICARE network.

• Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.
• Position will require attendance at DHA BHCF Training.
• Completion of DHA BHCF training and demonstration of BHCF Core Competence must be completed within 3 months of hire (unless DHA is unable to provide the training within 3 months).
• This position requires Basic Life Support (BLS) training certification from, or in accordance with, the American Heart Association and American Red Cross.
• This position has a mandatory seasonal influenza vaccination requirement and is therefore subject to annual seasonal influenza vaccinations.
• A Personnel Security Investigation is required.
• Selectees will be required to sign a statement (Condition of Employment) consenting to seasonal influenza vaccinations or must provide a recognized exemption.
• A pre-placement medical examination is required.
• Applicants must be able to fulfill non-privileged professional credential requirements.
• TDY may be required occasionally.
