Job Overview
We are seeking a Substance Use Disorder Case Manager to join our team and provide compassionate, professional support to individuals impacted by substance use disorders (SUD) and co-occurring mental health challenges. The ideal candidate will play a crucial role in providing support and guidance to individuals and families facing various challenges. This position requires a strong understanding of mental health, crisis intervention, and effective communication skills to facilitate positive outcomes for clients.
The Case Manager will work collaboratively with clients, families, and other professionals to develop comprehensive care plans tailored to individual needs.
- Conduct thorough patient assessments to identify needs and develop personalized care plans.
- Provide individual and group counseling using evidence-based practices such as Cognitive Behavioral Therapy and Motivational Interviewing.
- Implement crisis intervention strategies to support clients in acute situations.
- Facilitate access to community resources and services, including home care and hospital services.
- Maintain accurate medical documentation, including patient records, treatment plans, and progress notes in compliance with HIPAA regulations.
- Collaborate with multidisciplinary teams to ensure comprehensive patient care.
- Engage in research activities related to behavioral health and case management practices.
- Monitor client progress and adjust treatment plans as necessary based on ongoing assessments.
- Provide education on mental health issues, addiction counseling, and behavioral management techniques.
- Bachelor’s degree in Social Work, Psychology, or a related field preferred; relevant experience may be considered in lieu of advanced education.
- Proven experience in case management, crisis management, or related fields within mental health settings.
- Strong skills in psychotherapy techniques including Behavioral Therapy, Group Therapy, and Child & Family Counseling.
- Experience working with diverse populations facing issues such as PTSD care, addiction medicine, and general mental health challenges.
- Excellent communication skills with the ability to build rapport with clients from various backgrounds.
- Knowledge of data collection methods for clinical research purposes is an asset.
