Location: City of Syracuse
A community support organization in Syracuse seeks a Case Manager to provide person-centered assistance to reentry parents. Responsibilities include developing individualized case plans, documenting progress, and coordinating support services. The ideal candidate has experience in case management, excellent communication skills, and a BA/BS in relevant fields. Requirements include a valid NYS Driver’s License and the ability to transport participants.
