Full-time or Part-time post-acute care role for an NP or PA providing primary care services in skilled nursing facilities. This position offers strong clinical support, autonomy, and uncapped earning potential in a collaborative care model.
Position Highlights
? Full-time, 5 days/week rounding role
? Post-acute care in skilled nursing facilities
? No primary night call
? Fee-for-service compensation model
? Comprehensive onboarding with dedicated clinical mentor

Key Responsibilities
? Perform patient assessments, H&Ps, and care planning
? Manage acute and chronic conditions
? Coordinate care with physicians and interdisciplinary teams
? Educate patients and families
? Maintain timely and accurate documentation
