Overview
A growing post-acute care practice in the Syracuse, New York area is seeking a Nurse Practitioner or Physician Assistant to join a collaborative, well-supported team. This is a full-time opportunity (5 days/week) providing primary care services in skilled nursing facilities, offering strong clinical autonomy, structured onboarding, and uncapped earning potential in a mission-driven environment.
You’ll care for a diverse patient population, work closely with physicians and interdisciplinary teams, and play a meaningful role in improving outcomes during critical transitions of care.
Position Details- Full-time role rounding in skilled nursing / post-acute care settings
- Fee-for-service compensation model with no cap on productivity income
- Estimated annual compensation: $138,000–$164,000+
- Comprehensive 3-month onboarding program with a dedicated clinical mentor
- Ongoing support from experienced post-acute care clinicians and leadership
