Job Summary
The RN Navigator is a Registered Nurse whose clinical nursing experience guides patients and their families to informed decision‑making. The RN Navigator assumes responsibilities and accountability for planning, guiding and evaluating the delivery of patient care, monitors patients’ progress throughout treatment, intervenes as needed to ensure compliance and optimum care, and collaborates within a multidisciplinary team to facilitate timely, appropriate screening, diagnosis, therapeutic intervention, treatment and supportive care across the health‑care continuum.
The RN Navigator assumes responsibility for coordinating all resources necessary to provide care, utilizes advanced expertise to identify, implement and improve processes in order to provide an integrative model of care, while assuring the highest level of patient satisfaction. The RN Navigator provides patient education regarding disease process and treatment throughout the continuum, demonstrates and promotes professional development, and maintains a professional, safe and therapeutic environment for rendering care.
NYS licensed/registered or eligible RN required. Bachelor’s degree in Nursing or a health‑care related field with three (3) years post‑licensure RN experience (preferably in specialty) OR an AAS in Nursing with five (5) years post‑licensure RN experience (preferably in specialty) required. Excellent written/oral communication and interpersonal skills required. Current BLS certification through the American Heart Association or the American Red Cross is required by time of appointment.
Preferred QualificationsThree (3) years as a Certified Nurse/Patient Navigator in specialty area preferred.
Work DaysMonday – Friday 8:00 am – 4:30 pm in office position.
Department is accepting both full‑time and part‑time applicants.

G7: $27.00 – $44.36
