OverviewPosition summary and duties for a Licensed Practical Nurse in the Upstate Family Medicine setting. Reports directly to the practice's Nurse Manager. Provides prescribed medical treatment and personal care services under the supervision of a licensed medical practitioner or health professional, working within full scope of practice while ensuring high quality and safety.
Responsibilities- Review and prepare patient charts prior to appointments.
- Perform duties related to patient appointments and medical records, such as taking vital signs, noting problems, medications, pharmacy and health history.
- Maintain Epic EMR proficiency for routine clinical tasks (orders, changing providers, scheduling, recording administered injections, and quick disclosures).
- Enter orders into EMR and obtain insurance authorizations for tests and DME as indicated in provider notes/orders on the patient visit date.
- Act as liaison between patients, providers, office staff, and outside contacts; support smooth provider workflow, solve problems, and notify staff and patients of delays.
- Collect samples, perform routine lab tests, and administer medications.
- Clean and restock rooms; notify Team Leader or Nurse Manager of supplies to be ordered.
- Collaborate with Team Leader, Nurse Manager, and/or secretary staff to address chart and scheduling issues.
- Assist providers with procedures; prepare patients as required; anticipate provider and patient needs.
- Monitor patient satisfaction; report complaints to Nurse Manager or Practice Manager to maximize satisfaction.
- Sterilize instruments using proper procedures and equipment (e.g., autoclave); adhere to practice policies and procedures.
- Maintain current knowledge of health care issues, practice standards, and CPT/ICD-10 codes.
- Conduct symptom-based calls in Epic and refer to RN or provider for determination.
- Initiate medication authorizations.
- Handle patient calls directed to the assigned nurse pool and document in EPIC.
- Perform all other duties as assigned.
NYS licensed/registered LPN. Current BLS certification through the American Heart Association or the American Red Cross is required. Experience with electronic medical records. Familiarity with medical office procedures and standards of medical care.
Preferred QualificationsEpic experience preferred. 1 year of directly related experience preferred.
Work ScheduleMonday–Friday 8:00 am – 4:30 pm with potential for evenings and weekends in the future.
Part-time or full-time position available. 24–40 hours per week based on candidate preference. Candidates may select a 24, 32, or 40 hour position.

A $3,000 sign-on bonus is offered (half paid at start, half after 6 months of satisfactory service). Salary range: G5: $20.48 – $34.22 per hour.
