RoleOur Palliative Medicine is currently hiring a Nurse Practitioner
. The Palliative Medicine Service Nurse Practitioner works as a member of a multi-disciplinary clinical team. Responsibilities include working directly with patients and their families/support network and collaborating with a patient’s attending physician, specialty healthcare providers, nursing staff, and ancillary staff to identify and facilitate goals of care. The Palliative Medicine Service NP may be called upon to have advance care planning conversations with patients and/or their decision makers, including completion of any associated MOLST or HCP forms, or to provide assessment and recommendations regarding complex pain or other symptom management.
In the case of the latter, the NP will develop treatment recommendations in collaboration with the Palliative Medicine Service Physician. In addition, the Palliative Service NP will not infrequently need to determine a patient’s capacity to make healthcare decisions for themselves, facilitate family meetings, facilitate referrals to Hospice and Francis House, and implement comfort care orders.
Hourly Rate: $55.00 - $69.00 / hour

Schedule:
20 hours a week, can be 8-hour or 10-hour shifts.
- Obtaining health histories and performing physical examinations.
- Ordering appropriate diagnostic tests and treatment regimens.
- Providing medical education to the patient, family, nursing staff, as well as being available for consultations and crisis intervention.
- Working closely with the patient/significant others/families and all members of the multidisciplinary team to develop an appropriate plan of care and communicate it effectively.
- The Nurse Practitioner will practice within the guidelines of the written practice agreement and practice protocols.
- Current New York State license as a Registered Professional Nurse.
- Current New York State License as an Adult, Family Nurse Practitioner or Specialty area
- Minimum of two (2) years RN experience in critical care nursing.
- Master of Science Degree in Nursing or its direct equivalent, preferred.
- ACLS certification required.
