Registered Nurse for the Licensed Agency Case Manager (LHCSA) is responsible for providing nursing assessments of clients in their homes and over the telephone, providing resources to clients, families and aides in regards to agency and community programs, coordination of services, documentation in medical records in accordance with Quality Standards, and under the guidance of the Clinical Operations Manager supporting Onondaga, Oneida, and Madison Counties.
PerformanceResponsibilities and Standards:
- Conduct nursing assessments and supervision of home health aides
- Case‑manage a diverse client caseload
- Develop a plan of care based on the client assessment to meet their needs покупателей via an intra‑agency approach
- Ensure that documentation meets quality standards and is completed as per agency policy
- Understand and follow the nurse code of ethics, conduct and standards of practice
- Communicate and coordinate with internal and external resources to meet client needs
- Maintain established productivity expectations
- Provide education to clients, families and aides as needed solares
- Assist in Immunization Clinics when needed

- Attend team meeting as required
- Supervise home health aides in performing designated tasks related to patient care and per the care plan

- Assist with home health aide training program when needed
- Assist with aide evaluations as directed
- Maintain confidentiality
- Demonstrate knowledge and use of infection control practices
- Demonstrate knowledge of federal, state, and local government rules and regulations
- Adhere to quality improvement standards
- Maintain personnel requirements as current
- Demonstrate nursing skills competency through periodic evaluations
