• RN Case Manager / Utilization Review / Clinical Documentation Improvement (CDI)

• Seeking MDS (Minimum Data Set) experienced RN

• Acute care hospital–based case management role

• Active New York State RN License

• BLS Certification (must be current and able to be recertified)

• Minimum 1.5 years of recent experience (within the past 2 years) in an acute care hospital of similar size/acuity

• Travelers must have experience in equal or higher-level hospitals or trauma centers

• EPIC EMR experience required

• Prior acute care hospital case management experience required

• Utilization Review

• Clinical Documentation Improvement (CDI)

• MDS (Minimum Data Set) assessments

• Interdisciplinary care coordination

• Discharge planning & payer communication
