Opportunity OverviewFull-time inclusion in a post‑acute care role for an NP or PA providing primary care services in skilled nursing facilities. This position offers strong clinical support, autonomy, and uncapped earning potential within a collaborative care model.
Position Highlights- Full-time, 5 days/week rounding role
- Post-acute care in skilled nursing facilities
- No primary night call
- Fee-for-service compensation model
- Comprehensive onboarding with dedicated clinical mentor
- Population health and ACO participation with shared savings potential
- Perform patient assessments, H&Ps, and care planning
- Manage acute and chronic conditions
- Coordinate care with physicians and interdisciplinary teams
- Educate patients and families
- Maintain timely and accurate documentation
- Competitive FFS compensation with uncapped earnings
- Estimated annual compensation: $138,334–$164,001 (uncapped productivity)
