Oneida, Oswego, or Onondaga County. Up to $15,000 sign‑on bonus. Fleet vehicle program available.
At Nascentia Health, you’ll provide skilled, compassionate care to children with complex medical needs in the comfort of their homes, helping families feel supported and young patients thrive. Home visits are scheduled Monday‑Friday, 8:00 AM‑4:30 PM. There is no on‑call requirement. This full‑time position offers flexible scheduling with a full benefit package!

What you’ll do as a Pediatric RN case manager:
- Deliver safe, family‑centered pediatric nursing care in the home
- Serve as the primary case manager, coordinating care with the interdisciplinary team
- Develop and implement individualized plans of care
- Provide family teaching, support, and education
- Ensure accurate, timely documentation (documentation time built into the home visit)
- Supervise and provide oversight of paraprofessional staff, including home health aides, ensuring tasks are performed safely and according to the patient’s plan of care
- Maintain compliance with all infection control and regulatory standards
- Stay current with clinical best practices
- Current NYS registered nurse license (accredited program)
- BSN preferred (tuition reimbursement available!)
- Valid driver’s license, auto insurance, and reliable vehicle
- Self‑directed, organized, and comfortable working independently
- Pediatric or maternal/child health experience a plus
- Strong communication and computer skills
- Ability to sit or stand for long periods
- Ability to lift/carry 25–40 lb and assist with transfers up to 150 lb
- Comfortable with bending, kneeling, climbing stairs, and driving in all weather
- Ability to use a phone, laptop, tablet, and electronic medical record
- Category III blood borne pathogen exposure (rare contact with blood/body fluids)
Competitive salary
:
This position is an S03 exempt position with a min‑max rate of: $75,250 – $87,138.92 annually.
