Location:

Oneida, Oswego, or Onondaga County. Up to $15,000 sign‑on bonus. Fleet vehicle program available.

Home visits are scheduled Monday‑Friday, 8:00 AM‑4:30 PM. There is no on‑call requirement. This full‑time position offers flexible scheduling with a full benefit package!

What you’ll do as a Pediatric RN case manager:

• Deliver safe, family‑centered pediatric nursing care in the home
• Serve as the primary case manager, coordinating care with the interdisciplinary team
• Develop and implement individualized plans of care
• Provide family teaching, support, and education
• Ensure accurate, timely documentation (documentation time built into the home visit)
• Supervise and provide oversight of paraprofessional staff, including home health aides, ensuring tasks are performed safely and according to the patient’s plan of care
• Maintain compliance with all infection control and regulatory standards
• Stay current with clinical best practices

• Current NYS registered nurse license (accredited program)
• BSN preferred (tuition reimbursement available!)
• Valid driver’s license, auto insurance, and reliable vehicle
• Self‑directed, organized, and comfortable working independently
• Pediatric or maternal/child health experience a plus
• Strong communication and computer skills

• Ability to sit or stand for long periods
• Ability to lift/carry 25–40 lb and assist with transfers up to 150 lb
• Comfortable with bending, kneeling, climbing stairs, and driving in all weather
• Ability to use a phone, laptop, tablet, and electronic medical record
• Category III blood borne pathogen exposure (rare contact with blood/body fluids)

Competitive salary
:
This position is an S03 exempt position with a min‑max rate of: $75,250 – $87,138.92 annually.
