Overview

Job Title: Project Director, Social Care Network (SCN)

Reports To: Chief Program Officer (CPO)

The New York State Department of Health has established Social Care Networks (SCNs) as part of the 1115 Waiver Demonstration Amendment, and Forward Leading IPA (FLIPA) is the SCN Lead for the Finger Lakes Region. This region is comprised of 14 counties, including Allegany, Cayuga, Chemung, Genesee, Livingston, Monroe, Ontario, Orleans, Schuyler, Seneca, Steuben, Wayne, Wyoming, and Yates. The SCN’s aim is to enhance the delivery of social care services to Medicaid members by coordinating efforts among community-based organizations (CBOs) and other health care partners to create a more resilient, flexible, and accessible social care system that reduces health disparities and advances health equity.

The SCN will collaborate with CBOs and other health care partners, leveraging shared data and technology to coordinate social care services for Medicaid members to improve access, ensure reliable and timely referrals, streamline and track navigation and completed referrals in closed loop systems and enhance collaboration between social care service providers and other regional partners.

Job Summary

This position is responsible for directing the implementation and ongoing execution of the Social Care Network (SCN) in accordance with New York State Department of Health (DOH) requirements under the 1115 Waiver. The Project Director ensures that SCN deliverables are completed on time, within scope, and in compliance with state guidance and contractual expectations. The Project Director serves as the primary operational lead for the SCN and is responsible for translating executive strategy into actionable workplans, coordinating cross-functional implementation efforts, and maintaining accountability across SCN teams and contracted partners.

This role provides direct supervision of SCN staff that support network development, engagement with and success of contracted entities, training, and performance to support readiness, compliance, and continuous improvement.

Forward Leading IPA (FLIPA) is a nonprofit membership association of safety net providers working in partnership to provide the highest quality integrated healthcare to historically underserved populations in Upstate New York since 2017. FLIPA is renowned for its commitment to integrating primary care, behavioral health, and social care needs. Our growing membership serves individuals across more than 27 counties and includes federally qualified health centers (FQHCs), behavioral health providers, and a rural health network consisting of eight county public health departments.

Duties / Responsibilities

Program Leadership

  • Provide day-to-day operational leadership for the SCN, ensuring alignment between executive direction and frontline implementation activities.
  • Define and manage SCN scope, objectives, and implementation priorities in alignment with DOH guidance, SCN Program Manual requirements, and 1115 Waiver standards.
  • Develop and oversee implementation plans, including milestones, timelines, and resource needs, in collaboration with executive leadership.
  • Monitor SCN performance and outcomes to identify trends, gaps, and opportunities for improvement.
  • Support strategic planning for post‑1115 Waiver sustainability, including operational readiness, partner capacity development, and network continuity.
  • Coordinate activities across departments, partner organizations, and geographies to support consistent execution within diverse organizational and community contexts.
  • Forecast and manage program resources efficiently in collaboration with executive leadership.
