Monday – Friday: 8:30 a.m. – 4:30 p.m.

CNY is continually growing and expanding our reach. Offering person-centered services that empower individuals of all ages and abilities to reach their full potential as part of our community. Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.
The Role
We are looking for a proactive employee to utilize a variety of methods, including The Social Care Networks Health-Related Social Needs (HRSN) screening tool, to assess and support eligible participants.
Responsibilities- Collaborate with Agency programs to screen all Access
CNY participants for health-related social needs - Contact individuals and/or designated supports, i.e. family, staff, etc., in order to provide information about community resources that address identified needs
- Make follow-up contacts to determine progress made in addressing identified needs and provide additional information and/or support, as requested
- Use referrals from the Social Care Network to screen individuals in Central New York for health-related social needs
- Assist individuals with accessing community supports by utilizing the designated online referral platform in order to connect participants with available services
- Provide care management and housing services for eligible participants referred to Access
CNY, via the online platform - Complete all program documentation in a timely manner and per Agency standards, as requested by funding source(s); documentation includes, but is not limited to: service plans, service notes, financial, and program outcomes, etc.
- Complete incident report forms thoroughly and in a timely manner, per Agency standards
- Demonstrate flexibility, politeness, and respect, in all interactions with individuals, families, community providers, and coworkers
- Conduct self in a professional manner
- Provide person-centered, quality services, with a trauma-informed approach
- Ensure compliance with federal, state, and local employment laws and regulations, as well as company policies, including but not limited to corporate compliance, personnel policies and HIPAA
- Perform Other Duties as Assigned
High school diploma/GED required. Associate degree in a human services field, preferred. Must have knowledge of community resources. Excellent written and verbal communication skills, required.
Conditions of EmploymentMust have a valid NYS Driver License in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs.
