Flexible Work Schedules
ResponsibilitiesThe Hospitality Center Outreach Worker will Engage and support individuals and families through proactive community outreach. Assess immediate needs, connect individuals to shelter, housing resources, benefits, and supportive services including housing, treatment, and community-based supports. They will conduct street, shelter, and community-based outreach to identify and engage individuals and families in need of community support. Assess clients’ needs related to housing, safety, income, employment, food, clothing, and physical, substance or mental health concerns.
They will gather and evaluate relevant medical, psychological, and social information to inform service planning and referrals. Provide supportive counseling, crisis intervention, and case management to address needs and barriers. As well as, assist clients in developing and pursuing individualized plans to improve overall well-being and connect clients to community resources, benefits, shelters, housing programs, and other service providers; maintain collaborative relationships with partner agencies.
The Outreach Worker will complete required documentation, outreach logs, progress notes, and reports in accordance with agency and funding requirements. Maintain regular communication with supervisors and funding sources and promptly report safety concerns or critical incidents. Adhere to Catholic Charities policies, procedures, and all applicable funding guidelines. And perform other duties related to outreach, homelessness response, and community needs as assigned.
This Position is Full Time at 35 Hours Per week. Pay For This Position is $21.00 Per Hour.
QualificationsEducation and/or Experience Required at Entry:
- Associates’ Degree in Human Services related field.
- With three years’ experience working with vulnerable individuals
- Equivalent combination of education and relevant experience will be considered
Licenses/Certificate/Clearances
- Valid N.Y.S. Drivers License,
- registered and insured vehicle,
- N.Y.S. Child Abuse Registry clearance
Skills And Abilities
- Excellent oral and written communication skills.
- Proficiency with Microsoft Word
Working Conditions And Environmental/Physical Demands
- Mobility sufficient to drive to and participate in meetings at client’s homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals,
- Manual dexterity sufficient to work with children/adults in a challenging and active environment.
