Provide professional social work services to individuals with mental illness in a recovery-oriented environment as part of a multidisciplinary team.

• Provide crisis intervention, service linkages, and discharge planning.
• Develop treatment plans with individuals and the treatment team; monitor and evaluate progress and modify plans as needed.
• Determine discharge needs and appropriate community resources.
• Establish and maintain relationships with community providers to ensure continuity of care.

• Licensed Master Social Worker (LMSW) registered with New York State (eligible for hiring under NY HELPS program).
• Or, one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.
